Consequently, I may not have access to the entirety of my health information online in my personal health record. I also recognize that Sunrise Research Group reserves the right to restrict or discontinue my use of online patient services if I fail to adhere to these terms and conditions or at Sunrise Research Group's sole discretion.

I understand that the expected response time for electronic messages is 1 to 2 business days.
I am aware that I have opted into a service enabling electronic interaction with my clinician. Such interactions are not recorded, but images may be captured to aid in my care. I understand that I am prohibited from recording electronic interactions with my clinician.
Attachments or image captures may be used to facilitate communication with a clinician but should not substitute an in-person consultation or seeking emergency services.
For any urgent matters that I believe could immediately impact my health or well-being, I will visit the local hospital's emergency department, or dial 911.
I understand that my healthcare team may send secure e-messages through my account. These messages may contain crucial health and medical care information. It is my responsibility to monitor these messages. By providing my valid and functional email address, I have enabled Sunrise Research Group to notify me of messages sent to my inbox. Maintaining accurate contact information with Sunrise Research Group's online patient services is my responsibility, and I will update my email address on my account as necessary.
Contents of Electronic Messages:
The most suitable type of electronic communication for your specific case should be agreed upon by you and your clinician. Using electronic communication for solicitation purposes is prohibited. All Sunrise Research Group-generated EHR (Electronic Health Record) content adheres to state and federal statutes governing the security and confidentiality of health records.
Electronic Interactions:
Messages, attachments, or images you send via your online account or those captured during video interactions with your clinician may become part of your permanent health record. These will be accessible to current and future Sunrise Research Group staff involved in your care. If your Sunrise Research Group clinician is unavailable or out of the office, messages sent may be directed to other authorized clinicians within Sunrise Research Group to ensure a prompt response to your requests or questions. While you may receive Internet email notifications informing you of new messages in your account inbox, these emails will not contain sensitive health information.
